Legal Name Change Request

To request a legal name change, please use the following guidelines:

  • Faculty and staff should contact Human Resources regarding name changes.

  • Students can request a legal name change in one of these ways:

    1. In-person, by going to the University Registrar's office, Sears Library Building, Room 220.

    2. By mail, sending your request to the following address: Office of the University Registrar, Case Western Reserve University, 10900 Euclid Avenue, Sears Library Building, Room 220, Cleveland, OH, 44106.

    3. Electronically, by continuing with this form.

 

All requests require supporting documentation. Refer to the University Registrar's Name Change page for a detailed list of acceptable documents.
